Job Title: Case Management Education & Training Specialist (YASS Clinical Case Manager)

SAMMinistries’ Young Adult Stability and Support (YASS) Training Specialist will provide clinical case management support and crisis intervention services to families who are enrolled in SAMMinistries’ YASS program. The Case Management Education & Training Specialist will assess the needs of participants and coordinate and advocate for access to services that support stabilization of mental health and development of skills to foster long-term housing stability and self-sufficiency. The Case Management Education & Training Specialist will provide support and training to Case Management staff. Case management services will be offered using a collaborative, client-driven approach and will utilize Trauma-Informed evidence-based practices such as: Harm Reduction and Motivational Interviewing.
Essential Job Functions and Responsibilities
  1. Conduct intake screening and psychosocial assessments, brief therapeutic interventions, connection to mental health and substance abuse treatment, and crisis intervention as needed. Communicate clinical assessments, interventions, and ongoing client contact through timely, thorough record keeping, contact forms, and verbal communication.
  2. Conduct joint case management sessions with other Case Managers from SAMMinistries’ YASS program, as needed.
  3. Work collaboratively with Case Management staff to ensure that individuals and families housed in the program are connected to resources that address problems underlying the current/recent experience of homelessness.
  4. Advocate for the needs of individual clients as well as for systemic change, whenever needed for the overall benefit of homeless individuals.
  5. Attend and actively participates in team meetings, demonstrating an ability to prioritize participant needs and provide needed assessments and services in an ethical manner.
  6. Identify, develop linkages with, and coordinate access to services in the community and ensure active follow-up occurs.
  7. Work collaboratively with staff of other community agencies to establish and maintain partnerships that maximize benefits and resources for SAMMinistries Program participants.
  8. Provide clinically and ethically sound guidance to other SAMMinistries Case Managers, to complement their work with clients regarding obtaining and maintaining housing, obtaining employment, income, etc.
  9. Partner with case managers in their development of individual service plans for program participants. Provide resource information, clinically focused guidance, and work with Case Managers to meet with individual clients/families, as needed to assess and to assist in the service planning process.
  10. Transport SAMMinistries Program participants in personal vehicle, as needed.
  11. Provide crisis intervention for program participants, including assisting Case Managers with crises involving mental health, assessing for potential danger of participants harming themselves or others, assessing for danger in situations involving domestic violence, etc.
  12. Complete necessary documentation and tracking of activities, in accordance with all agency and grantor requirements.
  13. Understand and maintain client confidentiality and record-keeping, in accordance with HUD, SAMMinistries, and HIPAA privacy practices.
  14. Enter data into HMIS in a timely and accurate manner.
  15. Provide and promote resources for after-hours crisis intervention and support Case Managers in developing realistic and accessible crisis response plans for their clients
  16. Perform other duties, as assigned.

Education and/or Experience
  1. Master’s Degree in Social Work and licensed as LMSW or LCSW. Bilingual (English/Spanish) preferred.
  2. Minimum two years of experience working with individuals experiencing homelessness, preferred, or a minimum of three years’ experience working with vulnerable populations in a closely related field.
  3. Must understand and use approaches and models that are aligned with SAMMinistries YASS program, including Housing First and Trauma Informed Care. Experience with principles and approaches of Harm Reduction and Motivational Interviewing.
  4. Must expand and improve case management skills through formal training, reading and on-the-job experience.
  5. Must be able to prioritize multiple job responsibilities to produce a high volume of work in a short period of time.
  6. Must communicate orally and in writing in a professional, clear, and accurate manner.
  7. Must possess excellent interpersonal skills and be able to work with project participants, agency staff, and community partners, productively.
  8. Must be able to readily and regularly implement feedback as this position is being developed to best support SAMMinistries programs.
Language Skills
Bilingual (English/Spanish) is preferred.
Mathematical Skills
Ability to calculate figures and amounts such as addition, subtraction, division and percentages.
Certificates, Licenses, Registrations
Class C driver's license. Licensed social worker or equivalent licensure desirable.
Work Environment
The employee may be in contact with individuals and families in crisis who may be ill, using alcohol and drugs, and who may not be attentive to basic personal hygiene, health and safety practices. The employee may experience a number of unpleasant sensory demands associated with the client’s use of alcohol and drugs, and lack of personal hygiene. The employee must be ready to respond quickly and effectively to many types of situations, including crisis situations and potentially hostile situations. The noise level in the work environment is usually moderate in an office setting. Sometimes work may become stressful when working under pressure. May be required to work flexible hours to include evenings and weekends.
