Duties:

• Perform physical examinations and health assessments on patients
• Collaborate with healthcare team members to develop and implement patient care plans
• Educate patients and their families on disease prevention and management
• Train and educate staff on HHA requirements and updates requirements as set forth by SCDHHS
• Formulate, track and conduct HHA annual in person trainings and files as per state regulations
• Conducting patient visits as needed per client centered approach and/or state requirements
• Observe HHA and complete required assessment upon due date
• Creating and maintaining Patient centered care plans as per state/Medicaid requirements
• Approving and signing off on HHA task sheets as needed.

Qualifications:

• Must be a SC Registered Nurse with a valid license
• Proficient in medical terminology and documentation
• Strong knowledge of nursing principles and practices
• Minimum 3 year experience in providing home care services and assessments administration
• Excellent communication, documentation and interpersonal skills
• Attention to detail and strong organizational skills

Job Types: Contract, PRN, Per diem

Pay: $40.00 - $45.00 per hour

Expected hours: 5 – 10 per week
