We are seeking compassionate and dedicated individuals to join our team as Community Habilitation Workers.

Community Habilitation Workers provide direct support and assistance to individuals with developmental disabilities, helping them to acquire and enhance skills necessary for independent living and community integration. The ideal candidates will be committed to empowering individuals to achieve their goals and lead fulfilling lives.

Responsibilities:

• Provide one-on-one support and instruction to individuals with developmental disabilities in their home and community settings. Potential goals may include:
• Assist individuals in developing and practicing daily living skills, such as meal preparation, household chores, personal hygiene, and budgeting.
• Support individuals in accessing and participating in recreational, social, and community activities, fostering meaningful connections and relationships.
• Promote independence and self-advocacy skills by encouraging decision-making and problem-solving opportunities.
• Document individual progress, goals, and activities accurately and in a timely manner, in accordance with agency policies and regulations.
• Communicate effectively with individuals, families, caregivers, and other team members to ensure consistent and coordinated support.
• Maintain a safe and supportive environment for individuals, recognizing and addressing any health, safety, or behavioral concerns.
• Collaborate with community resources and service providers to facilitate access to additional supports and resources as needed.
• Participate in ongoing training and professional development opportunities to enhance skills and knowledge in the field of developmental disabilities and community habilitation.

Job Type: Part-time

Pay: $16.00 - $20.00 per hour

Expected hours: 10 – 30 per week
