The Cypress at Midtown is looking for an experienced Director of Social Services & Admissions to join our awesome, energetic team! Don't miss this opportunity to become part of a team that prides itself on a positive, fun culture!

Summary/Objective In keeping with our organizations goal of improving the lives of the Residents we serve, the Director of Social Services manages the day-to-day operations of the Social Services Department and responsible for the overall administration, coordination and evaluation of the social services function to meet and maintain the mental and psychosocial well-being for each Resident.

They are also responsible for the entire admission process of Residents as well as the marketing program for the facility, develops and maintains professional relationships with the surrounding medical community to ensure steady referral sources for the facility. This position also reviews medical, financial and other records with appropriate department heads of potential Residents to ensure they are appropriate for the facility.

Essential Functions

- Responsible for keeping up-to-date evaluation documentation on each Resident's activities at the facility which complies with Federal, State, and Local regulations.

- Attends scheduled care conferences and identifies and interprets psychosocial needs of Residents for inclusion in the care plan, addressing in particular issues of schedule, treatment and environment that express and reinforce individuality and identity.

- Assists with the coordination, intake, admission and Resident transfer within the facility.

- Coordinates the inspection of Resident's belongings to ensure they are properly labeled and inventoried.

- Attends Interdisciplinary Team and Unit Care conferences, Assesses Residents for psychosocial groups, and supervises implementation and documentation.

- Mediates issues that arise among Residents, families and staff.

- Provides linkage with appropriate community resources by maintaining knowledge of other systems, making referrals and identifying unmet needs (e.g., recreational transportation, adaptive phone equipment, etc.)

- Develops a comprehensive discharge plan in collaboration with Resident, Resident's families, multidisciplinary staff, and referral agencies.

- Responsible for operating the Social Services Department within budgetary guidelines and limitations.

- Responsible for training and educating staff members.

- Maintains the comfort, privacy and dignity of Residents and interacts with them in a manner that displays warmth, respect and promotes a caring environment.

- Attend and participate in departmental meetings and in-services as directed.

- Coordinates tours of the facility with perspective Residents, families and referrers either personally or with appropriate interdisciplinary team members.

- Assists Residents and family members with completing the admission process by assuring all admission paperwork is completed and necessary financial and medical records are obtained.

- Develops and maintains referral base.

- Assists with the development and implementation of long-range and short term marketing plans.

Education (Preferred)

Bachelors degree in social work or similar field.

Two years experience in a social worker role, preferably in a health care setting.
