NTSOC is looking for a compassionate LPN to provide high quality nursing care for our pediatric and adult clients to improve their quality of life at home. Job Type: Full time and Part time Compensation: $24.00-$33.00 per hour dependent upon experience. Essential Functions:

  • Provide one on one care to a single client in their home setting and while in the community.
  • Implements the clients physician ordered plan of care and nursing interventions while adhering to agency policies.
  • Perform assessments to monitor the health status of the client.
  • Notify the physician and Field Nurse Case Manager of any changes in the clients condition.
  • Coordinate care with the Field Nurse Case Manager and other members of the clients multidisciplinary care team.
  • Administer medications and treatments as ordered.
  • Document performed interventions and treatments.
  • Assists clients with activities of daily living such as bathing, toileting, dressing, and grooming.
  • Collaborate with the care team on implementation and updates to the clients plan of care.
  • Monitor client progress toward home health care goals.
  • Educate the client and family regarding the plan of care and nursing care needs.

Qualifications: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. Minimum Education : Graduate of an Accredited Nursing Program required. Minimum Experience: 1-2 years home health care experience preferred and 1 year of nursing experience in a health care setting preferred. Strong communication skills. Strong computer skills. Licenses/Certifications : Colorado LPN license in good standing with the Department of Regulatory Affairs. BLS required. Up to date car insurance and driver license will be required.
