Summary Description:

The OnTrack Clinician will be part of a small, multidisciplinary team providing coordinated and assertive treatment to adolescents and young adults (16-30 years old) who are within two years of experiencing the first onset of psychosis. The clinician will serve as the point person for participants and their families, offering supportive therapy, psychoeducation, behavioral interventions, and family support. The role also includes engaging with the participant's social and community environments to promote effective transitions and care continuity. Training for this role is provided through the statewide OnTrackNY Initiative at the Center for Practice Innovations at Columbia Psychiatry and the New York State Research Foundation for Mental Hygiene.

Key Responsibilities:

  1. Serve as the primary point of contact for participants and their families.
  2. Provide psychoeducation on first-episode psychosis (FEP) and the goals of the OnTrackNY program.
  3. Offer supportive therapy and behavioral interventions, with an emphasis on family support.
  4. Conduct clinical assessments to determine participants' social functioning and substance use using a shared decision-making (SDM) framework.
  5. Provide training in social skills, substance abuse treatment, behavioral activation, coping skills, and psychoeducation.
  6. Connect participants and families with necessary services and support.
  7. Develop and modify treatment plans collaboratively with clients using shared decision-making.
  8. Provide services off-site as needed, based on participant and family preferences.
  9. Conduct needs assessments, safety planning, and wellness management planning with participants.
  10. Facilitate participants' transition from the team to other services and supports, ensuring stable links upon discharge.
  11. Attend weekly team meetings.
  12. Perform other designated and related duties.

Key Competencies:

  1. Experience in an Article 31 Mental Health Clinic (preferred).
  2. Experience in outreach to underserved populations and working with individuals with serious and persistent mental illness.
  3. Knowledge of city, state, and federal guidelines and laws affecting mental health treatment and reporting procedures.
  4. High degree of computer literacy and familiarity with Electronic Health Record systems.
  5. Recovery-oriented, evidence-based clinical practice (preferred).
  6. Experience with dually diagnosed individuals and diverse populations (preferred).
  7. Understanding the impact of culture on mental health practices.
  8. Demonstrated commitment to diversity, equity, inclusion, and cultural humility.

Key Performance Indicators (KPIs):

  1. Quality and effectiveness of therapeutic and supportive services provided.
  2. Level of engagement with participants and families.
  3. Success in establishing and maintaining connections with relevant services and supports.
  4. Timeliness and accuracy of clinical documentation and assessments.
  5. Active participation in team meetings and adherence to program protocols.

Requirements:

  1. New York State licenses: LMSW, LMHC, Psychologist, or LCSW.
  2. Legally eligible to work in the United States.
  3. Ability to work independently and maintain confidentiality.
  4. Ability to participate in multiple hours of live video training
  5. Bi-lingual Spanish (preferred).
  6. Basic Community System Knowledge and Microsoft Office proficiency
  7. Valid Driver’s License with a satisfactory driving record.
  8. Clearance through fingerprinting and various state and federal registries (e.g., NYS Justice Center, SEL, NYS Medicaid Exclusion List, DCJS NYS Sex Offender Registry, NYS OCFS).
