The Clinical Assessor is primarily responsible for Bios, CSSRS assessments, eating disorder assessments, and all of the follow through from these assessments: safety contracts and informing staff of monitored eating. He/ She will conduct chart audits and will head the Quality Assurance committee. He/ She will do weekly chart audits and will assist in signing off on charts. The Clinical Assessor is responsible for training new staff and interns on the documentation requirements.
  • Review assessments from referring facilities and note problems/issues that require further attention and integrate this information into patient's current course of treatment.
  • Contact referral source and other key individuals no less than weekly to gather additional information as necessary and to update on progress as necessary.
  • Complete ASAM Forms (admission, continued stay, transfers and Discharge) for each level of care. Documenting Criteria.
  • Gather comprehensive diagnostic information through psychosocial assessment interview.
  • Develop an Integrated Summary, bringing together information from all assessments to identify problem areas that may affect treatment and/or need to be addressed in treatment planning.
  • Develop a comprehensive problem list that reflects patient's individual issues as described in the assessments and Integrated Summary and ensure that the list is carried to the treatment plan.
  • Present psychosocial and diagnostic information to treatment team in a clear, concise manner.
  • Maintain clinical records according to program policies and those of licensing and accrediting agencies.
  • Ensure that documentation is timely, consistent, clear and articulate
  • Keep clinical records secure at all times.
  • Complete CSSRS assessments
  • Complete Eating Disorder Assessments
  • Complete Safety Contracts
  • Inform staff of pertinent client issues from assessments
  • Review/ sign documentation of unlicensed staff.
  • Oversee the QA committee-assign charts for Audit and maintain the QA meeting binder
  • Complete chart audits and inform staff of deficiencies
  • Fill in for clinical staff members as needed
  • Train/ mentor staff and interns on documentation/ assessments
  • Demonstrates ability to work with Substance Abuse clients.
Required Experience, Education and License
  • Licensed Clinician in the state of Florida required
  • Master's Degree in Social Work, Mental Health Counseling, or Marriage and Family required
  • Three to Five Years of work experience in substance abuse and/or mental health field
  • CAP or state certification preferred.
  • Strong proficiency in Communication & Written skills.
  • Knowledge of State & Federal Statutes Regarding Patient Confidentiality laws.
  • Knowledge of Drug-Free Workplace Policies.
  • Knowledge of Workplace Violence.
  • Knowledge of Corporate Integrity & Compliance Program.
  • Demonstrates knowledge of Substance Abuse experience.
