Job description: We are looking for a registered professional physical therapist who provides coordinated care to patients of all age groups. Plans, implements and evaluates patient care plans to restore or maintain patient well-being. Plans, organizes and conducts physical therapy treatment program based on the medical referral and their evaluation. Collaborates with interdisciplinary team.
Educational Requirements:
  • Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program.
Licensing/Certification/Registration Requirements:
  • Current Texas State License as a Physical Therapist, current Texas Driver License, and current CPR.

  • Knowledge of durable medical equipment.
  • Strong interpersonal communication skills.
  • Self-direction with a strong sense of team commitment.
  • Ability to accept assigned duties and work in a cooperative manner with an interdisciplinary team.
  • Excellent verbal and written communication/documentation skills.
Physical Requirements:
  • Able to see/hear/speak sufficiently to participate in written/verbal communication.
  • Able to deal effectively with stress.
  • Able to use fine motor skills for patient assessment.
  • Able to stand, sit, bend, stoop, push, pull, and heavy lifting.
  • Able to operate a motor vehicle and travel to prospective patients residences.
Responsibilities:
  • Perform clinical assessment; Identify patient needs and appropriateness of patient.
  • Applies concepts of infection control and universal precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease.
  • Performs and oversees other team members, when applicable, in the performance of patient care that is consistent with patients' needs and desires.
  • Accepts clinical assignments that are consistent with education and competence to care for patients.
  • Assist/instruct patient/ family in transfer techniques appropriate to patient abilities. Instruct on use of equipment such as walkers, wheelchair, canes, etc., in a safe manner. Utilize appropriate therapy equipment specific to patient diagnosis and needs.
  • Document, prepare and complete a Plan of Care, progress notes, phone orders and other clinical record documentation on an ongoing basis. Revise the POC based on evaluation, ongoing assessment data.
  • Demonstrate commitment, professional growth and competency.
  • Must have a reliable means of transportation, proof of financial responsibility (car insurance and a valid state drivers license)
