Alvita is searching for compassionate and reliable RN's throughout both New York and New Jersey.Numerous client locations available making for an easy commute.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Essential duties and responsibilities for the Registered Nurse role include the following:

1. Performs the initial assessment visit
2. Regularly reevaluates the client needs, and coordinates the necessary services
3. Initiates the Plan of Care and necessary revisions and updates to the plan of care and the care plan
4. Provides services requiring specialized nursing skill and initiates appropriate preventative and rehabilitative nursing procedures
5. Informs the physician and other personnel of changes in the client condition and needs
6. Counsels the client and family/caregivers in meeting their needs
7. Prepares clinical and progress notes
8. Supervises and teaches other nursing personnel and home health aides as appropriate
9. Participates in continuing education programs

SUPERVISORY RESPONSIBILITIES:
This job has no supervisory responsibilities.

Qualifications:

• Must have a valid New York State RN license
• Must have updated medicals/vaccinations or be willing to obtain
• Completed drug screen or completed Habituation Form
