We are rapidly growing and are looking for a motivated Community Outreach Coordinator to grow with us. Bloom Health Centers provides comprehensive behavioral health services delivered by a group of competent, independent practitioners. We diagnose and treat children, adolescents, adults, and the geriatric population. Psychological testing and neuropsychological testing are available for a variety of disorders, including ADHD, brain injuries, head trauma, and memory problems.

About the Role

We are expanding and seek a talented Referral Program Coordinator to join our team.

As the Referral Program Coordinator, you will cultivate existing & new partnerships with practices, payors, and all other vendors intended to grow the company. You will collaborate with the Community Outreach Coordinator and members of leadership to manage relationhsips with referral partners and ensure that patients referred by partners adequately receive their intended services.

Job Responsibilities

• Meet weekly with Nurse Navigators regarding referrals
• Oversee community outreach services
• Schedule new patient referrals
• Attend weekly meetings with relevant company partners
• Collaborate with the Patient Care Experience staff to ensure all referral processes are being followed
• Schedule patients follow up appointments
• Maintain and track appropriate documentation to uphold knowledge on patient referrals
• Other duties as assigned by leadership

● Comfortable initiating contact with healthcare providers and front desk representatives.

● Skilled in establishing rapport. Excellent written and oral communication skills.

● Demonstrated initiative and attention to detail.

Qualifications

● Cumulative experience in marketing/sales, outreach, and/or building client relationships

● Bachelor’s Degree

● Computer experience with Office 365/One Drive and Microsoft Office products

● Basic knowledge of public relations

● Familiarity with CRM databases or willingness to learn
