Job Description: Responsible for all direct patient care. Assess each individual’s condition as you make rounds, be alert to any changes in the resident’s condition and response to medications and treatments. When accidents occur, record descriptions of bruises as to size and color, etc. Record size of the area of skin tears or lacerations. React to emergencies promptly and request necessary help, etc. Supervise in the dining room during meals, administer medications, and note reactions. Accompany physicians on rounds to answer any questions, receive instructions, and note resident care requirements. And take care of orders received. Answer call bells as necessary. Assist with patient care as necessary. Supervise staff to provide good care and keep the environment safe and clean.
Qualifications:
1. Graduate from an accredited school of vocational nursing
2. Current license from the State Board of Vocational Nurse Examiners for the State of Texas.
3. Three years of experience in general acute nursing, preferably with the older adult.
4. Two years of experience in a long-term nursing care facility.
5. Must be CPR certified.
6. Must be able to follow and understand oral and/or written English directions.
7. Must have computer knowledge.
PERFORMANCE REQUIREMENTS
MAJOR RESPONSIBILITIES FOR ALL SHIFTS:

Complete weekly summary on all residents.
RESPONSIBILITY: Provide and supervise nursing care to residents according to the physician’s orders and pursuant to the philosophy, goals and objectives, policies, and procedures of The Village at Incarnate Word.
KNOWLEDGE: Knowledge of general nursing theory and practice, preferably in the area of geriatrics for a better understanding of resident care problems. Knowledge of universal precautions.
SKILLS: Must be proficient in basic nursing skills, communication, ability to work independently.
MENTAL APPLICATION: Must be able to detect errors in own work, take corrective action, and protect the health and safety, and well-being of residents. Cope effectively in an emergency situation, and instill confidence in physicians, residents, families, and co-workers.
ACCURACY: Must be able to maintain consistent work performance. Absolute accuracy in administering medications and performing nursing procedures, with attention to detail.
DEXTERITY: Requires good eye/hand coordination, muscular control, and good aural acuity. Ability to operate resident equipment, and perform nursing procedures. Alertness in recognizing and identifying symptoms and emergency situations. Knowledge of proper body mechanics.
PHYSICAL DEMANDS: Sits, walks, bends, stoops, and lifts at least 50 pounds throughout the eight-hour detail. Also handling, fingering, pushing, pulling, carrying, reaching overhead, reaching forward, reaching low, kneeling, climbing, and balancing.
NURSING PROCEDURES: Perform nursing procedures including intake and output, measuring weight and heights, vital signs, specimen collection, and catheter care.
INFECTION CONTROL: Demonstrate knowledge of infection control in all resident care to prevent the spread of disease and infection. Adhere to strict infection control guidelines including hand washing and universal precautions.
SAFETY: Demonstrate knowledge of emergency policy and procedure. Perform all responsibilities in a safe and efficient manner. Follow established guidelines when using protective devices and equipment. Demonstrate awareness of potential hazards and is proactive in maintaining a safe environment for residents and staff.
ENVIRONMENT: Works in clean, well-lit, ventilated building, on the nursing unit, and in resident rooms.
HAZARDS: May be exposed to communicable diseases but must follow universal precautions. Possibility of strains due to moving residents or injury due to irrational behavior of residents. Possibility of cuts and minor burns from equipment falls, etc.
COMMUNICATING ABOUT RESIDENT CARE: Utilize plan of care in the delivery of resident care. Receive and give reports on resident’s condition at the beginning and end of each shift. Observe any change in resident condition. Communicate and cooperate with other members of the multidisciplinary healthcare team to accomplish the plan of care. Document accurately and on a timely basis on appropriate forms.
GUEST RELATIONS: perform all responsibilities with respect to interpersonal skills. Is pleasant, respectful, and courteous in all interactions with residents, families, staff, and other visitors to the facility. Act as a representative of the nursing center at all times.
