The CHHA home care RN is responsible for clinical assessments, development, oversight, and implementation of the approved patient care plan as developed by the patient/family and physician to maintain the patient safely at home. S/he will ensure that the care plan is appropriate and responsive to the patient’s health care needs. S/he will reevaluate clinical assessments and make any necessary changes to the care plan. Provides oversight of Home Health Aide services and participates in the interdisciplinary care delivery process for all patients admitted to the CHHA. This position requires the individual to work at least one weekend per month. To see patients in Lower Westchester OR Manhattan.
Job Type: Full-time
Pay: $52.19 - $60.43 per hour
Expected hours: 35 per week
