Primary Responsibilities

• Provide a minimum of 18 eligible participants each month with enrollment, redetermination, and navigation services throughout the Clackamas, Marion, Multnomah, Polk, and Washington counties.
• Attend outreach events and track and document the number of individuals reached.
• Follow up with participants in order to assist them with ongoing system navigation.
• Regular attendance is required.
• Other duties as required.

General Responsibilities

• Follow the protocol, policies, and procedures of the department and agency.
• Maintain accurate documentation on a consistent basis.
• Complete and submit reports as needed.
• Maintain confidentiality of client information and comply with HIPAA guidelines.
• Provide culturally responsive services that are equitable, respectful, and relevant to people from a wide variety of cultures, backgrounds, and orientations.
• Attend meetings and trainings as required.
• Regularly participate in professional development.

• Bilingual, bi-cultural English and Spanish is required.
• A high school diploma or equivalent is required.
• Must be a Certified OHA Assister or become certified within 6 months of taking the position.
• Have a Community Health Worker certification or become certified within 6 months of taking the position
