Our Saviour's Community Services is Hiring a Permanent Support Housing Lead Case Manager (hybrid) Near Minneapolis, MN

Position Title: Permanent Supportive Housing Lead Case ManagerOur Saviour’s Community Services (OSCS) provides dignified shelter and housing for single adults and basic education classes to adult English language learners in the Phillips Community of South Minneapolis. OSCS takes its name from Our Saviour’s Lutheran Church, which founded our programs in the early 1980s. In 2004, we became a legally and financially independent nonprofit. Our Saviour’s Housing operates a 21-bed emergency shelter and permanent supportive housing program for single adults over 18. The shelter provides a bed, three meals a day, and supportive resources to 10 men and 11 women every night of the year. However, people of all genders are welcome. The shelter is currently open to residents 24/7. The program has ten medical respite beds allocated to a local hospital. Our cohesive team works together to provide services that offer dignity and hope. Our Saviour’s Housing is a program of Our Saviour’s Community Services.

Our Saviour’s Housing operates a permanent supportive housing program for single adults in Hennepin County. The referrals are from Hennepin County’s Coordinated Entry System. The program aims to maintain housing for 100 single adults or couples that are 18 . The program finds housing in the community or uses our onsite housing buildings to house 10-12 participants. The program offers case management via housing support or housing stabilization. The participants have access to transportation provided by case managers and to our onsite food shelf.

This Role: The PSH Lead Case Manager provides strength-based advocacy and case management services for 10-15 long-term homeless adults in both site-based and scattered site housing. The PSH Lead Case Manager works collaboratively with other members of the Housing Team, as well as other Program staff members. The Lead Case Manager will support other case managers with their caseload. This position will also assist Case Managers with conflict resolution, housing stabilization services, assist with monthly reports, verify recertifications, assist with communication with the Department of Human Services and Hennepin County, and landlord relationships. 

 

This position is non-exempt, full-time, regularly scheduled at 40 hours per week, M-F, between 8:00 am to 5:00 pm. This role is within the PSH Team and reports to the Permanent Supportive Housing Program Manager. This position is a hybrid position that requires being onsite five days a week at least once a month.

 

Primary Duties and Responsibilities

  • Actively participate in building a culture of belonging within OSCS by centering equity in organizational and programmatic decision-making.
  • Assist participants with the housing move-in process – application, interview, and access to furniture.
  • Filter staff and client concerns and communicate the needs to the program manager 
  • Leads projects such as data collection, data implementation, data clean-up, client pep events and/or organizational projects
  • Supporting managers in enforcing existing and new organization and program policies and procedures . 
  • Onboard and provide residents with access to Housing Stabilization Services, including completing a Person Centered Plan for eligible participants.
  • Prepare and facilitate staff meetings that promote teamwork, collaboration, & creative problem-solving
  • Help train and onboard new staff.
  • Perform administrative duties such as entering data into CaseFlow, The Homeless Management Information System (HMIS), and Excel Spreadsheets and completing shelter intakes.
  • Facilitate conflict resolution and navigate residents’ complaints while working to promote a sense of community among residents and help navigate client grievances when needed.
  • Meet weekly with residents to establish an individual service plan, work toward greater self-sufficiency, and remove housing success barriers.
  • Perform case management duties that include but are not limited to: landlord tenant relationship communication and advocacy, crisis prevention, transporting clients, and supporting clients manage finances to be successful. 
  • Work cooperatively with internal and external stakeholders, such as Hennepin County, outreach organizations, and volunteers who provide services to our residents.
  • Formulate strong communicative and professional relationships with landlords.
  • Assist the Permanent Supportive Housing team with residents’ Housing Support Housing intake and Our Saviour’s Housing agreements. 
  • Complete and meet crucial deadlines such as entering detailed case notes, completing CaseFlow tasks, completing forms and paperwork, managing client budgets, submitting expense receipts to accounting, and other related requirements.
  • Prepare and file reports, as required by Hennepin County and other appropriate OSH housing reports. 
  • Attend regular OSH Case Management meetings, PSH office hours, and other OSCS-related meetings.
  • Attend and complete all required training, such as Hennepin County training for Housing Support land Housing Stabilization Services programs.
  • Various duties to be assigned and discussed as time permits and as the needs of OSH shift.

  • Able to communicate effectively in writing, by phone, and in person
  • Strong attention to detail and time management skills.
  • Ability and willingness to learn about homelessness and factors impacting our participants.
  • Ability to move around, drive to client locations, work on a desk and in front of a computer, sit for long periods, use keyboard and mouse, and perform repetitive motions such as typing, sorting, pinching, and other office routines.
  • Ability to speak, read, write, and understand people using Spanish, Somali, Arabic, Ukrainian, Russian, and Farsi/Persian languages (preferred but not required).
 

Experience and Qualification Requirements:

  • Experience working with clients with mental health diagnoses, substance use needs, and disabilities.
  • Experience working in a nonprofit, preferably Human Services industry, and six months of housing support experience (formerly Group Residential Housing or GRH).
  • Relevant personal, education, or work experience equivalent to 2 years
  • Demonstrate and maintain a good driving record, a reliable vehicle and insurance, and a valid driver’s license
  • Ability to pass a criminal background check and pass and maintain an MN Department of Human Services background study
  • Familiarity or trained in Homeless Management Information System (HMIS) and Casenote Software
  • Problem-solver and inclusive decision-maker to help field and manage routine and non-routine challenges and circumstances. 
  • Takes initiative, and works well independently and collaboratively.
  • Comfortable working with people from marginalized groups, people with chemical dependence disorders, and people with disabilities.
