NeoPath Health, Inc. is Hiring a Home Health Nurse Near Hastings, MN

The Home Health nurse is responsible for traveling to a patient’s home to administer home health nursing services to help patients to improve clinical outcomes and maintain their independence. Home health nurses will receive referrals from providers and will work closely with the Prairie Island Health Center clinical team and the Purchase and Referred Care Program to monitor the health and wellness of vulnerable community members. The individual(s) who hold this position will exemplify NeoPath’s commitment to working as a part of a highly engaged clinical team to provide an exceptional patient care experience. 

Job Duties

  • Monitor patient’s condition and assess their needs to provide the best possible care and advice.
  • Observe and interpret patient’s symptoms, behaviors, and communicate them to patient’s primary care physician. Perform physical examinations and diagnostic tests.
  • Collaborate with primary care physicians and clinical team to devise individualized care plans for patients. Collect patient history.
  • Perform routine procedures (blood pressure measurements, administer injections, administer medication and medication management, at-home IVs, wound care, assess health care needs and/or other needs, arrange for additional assistance etc.) and provide treatments in accordance with provider orders.
  • Immediately complete progress reports in the EHR for provider review.
  • Counsel and educate patients and their families on treatment plans.
  • Read and interpret patient information, making treatment decisions when appropriate.
  • Consult with clinical managers and providers to decide on the best treatment plan for patients.
  • Research ways to improve healthcare processes and improve patient outcome.
  • Inspect the facilities and act to maintain excellent hygiene and safety (decontaminating equipment, sanitizing surfaces, preparing beds etc.)
  • Seek medical care in emergencies.
  • Foster a supportive and compassionate environment to care for patients and their families.
  • Provider regular updates to the Prairie Island Health Center providers and PRC program about the patient’s health and wellbeing.
  • Receive referrals from providers and will work closely with the provider team to monitor the health and wellness of patients. 
  • Assist patients, PIHC providers, and PRC Program staff in navigating the various health care systems in the surrounding areas, fill out forms, make appointments, understand, and report client’s health and health conditions and be able to manage them, prevent disease or complications from disease and to make healthy decisions. 
  • Work closely with Community Health Representative(s) concerning patient care.

Qualifications and Education Requirements

Completion of Accredited nursing program.
Current MN Nursing License.
Current BLS Certification
2 years of experience working in a medical-surgical setting.
1 year’s experience working in a home health setting.
Current driver’s license issued by the State of Minnesota or Wisconsin
Phlebotomy experience preferred.
Experience in working with Native American Population with Indian Health Services and/or a Tribal Healthcare Organization preferred.

Limited Travel Required
Some evening and weekend work may be required.
This position is 12 hours/week
This position will maintain current CPR, BLS and other approved certifications.
This position will maintain background clearance as required by the employer and any collaborating agencies.
