OLD COLONY Y is Hiring a Youth Service Coordinator Near Buzzards Bay, MA

The primary role of the Community Coordinator (YSC) is to work closely with the case management team to ensure that youth are connected to services which align with their identified needs, strengths and assessed level of risk. Community Coordinators shall be cognizant of each youth’s assessed level of risk and shall work with the casework team to develop and support the most effective approach to community supervision and service coordination commensurate with each youth’s service need areas all while accounting for and providing opportunities to support each youth’s strengths. The Community Coordinator will support the casework team in ensuring that the most intensive monitoring and interventions are applied to those at highest risk for reoffending. The Community Coordinator will balance the intensity of his/her involvement in cases in coordination with the DYS case management team concentrating on matching needed services of all youth to their specific dynamic risk factors. The Community Coordinator is a direct report to the Senior Director, Community Services Network for compliance with Old Colony Y policies and employment expectations as well as meeting DYS Contract expectations. The Community Coordinator receives his/her youth related and duties from the on-site DYS District Manager.

The Community Coordinator will be primarily sited at a District Office and will also be responsible for working with DYS Casework Staff, clients and the local communities associated with linked satellite sites. S/he will accept and display the values of caring, honesty, respect and responsibility.

JOB FUNCTIONS:

1. PROGRAM/DEPARTMENT RESPSONSIBILITIES:

· Supports DYS casework staff in the execution, implementation and coordination of appropriate services and activities related to the needs and strengths of each youth as identified in his/her case management plan.

· Attends initial staffings, 90/30/30 day meetings, and post revocation meetings as requested by the District Manager.

· Assists the Caseworker in the development of a YLS/CMI Service Matrix – aligning the YLS/CMI domains with needed services in the community which match the youth’s level of risk.

· Identifies, coordinates and sustains relationships with community-based organizations and providers to ensure a menu of services and resources is established to meet the individual needs of each youth and family.

· Conducts home visits, monitors youth curfew, discharge planning, school visits, employment checks, and attendance checks in collaboration with the DYS caseworker or as supplemental to DYS caseworker visits as requested by the DYS District Manager.

· Identifies pathways for DYS youth and families to access benefits and services available to them through state agencies/benefits providers (such as DTA, DCF, schools, financial aid, WIC, Department of Labor, MA Rehabilitation Commission, SSI and Medicaid) and work with DYS casework staff to inform and assist them in connecting youth on their caseloads to these benefits.

· Plans and executes pro-social services’ activities and District Office events such as recreational outings, community service projects and family engagement activities with the support of DYS casework staff.

· Establishes relationships with community-based organizations and act as point of contact to broaden and sustain an in-depth network of resources to meet the individual needs of youth and families

· Assists in and supports the implementation of service-focused initiatives in the region as requested by DYS.

· Works with Assent of Ward youth; knowledgeable of the specific services and needs of the transitional-aged youth population.

· Contributes to the development, monitoring and execution of goals and subsequent services identified in each youth’s community-based case management plan and/or Acknowledgement of Assent of Ward form.

· Assiss the DYS caseworker in ensuring youth are prepared for discharge from DYS; participate in the 90-60-30 day DYS discharge planning process for each youth in coordination with DYS casework staff.

· Acts as a support and collaborative partner in the realization and operationalization of each youth’s identified goals as stated in their individual Empower Your Future (EYF) work.

· Infuses PYD focus into work with staff, youth, families, and providers.

· Enters required data into the DYS Juvenile Justice Enterprise Management System (JJEMS) maintaining documentation of contact with youth and families and youth participation in community-based programming and services.

· Provides quarterly/monthly/weekly reports to the PCM and/ or District Manager.

· Ensures there are food supplies on site for youth events and activities.

· Ensures there are supplies on-hand as indicated in each region’s Emergency Preparedness plan; is knowledgeable of resources in the communities served by the District Office to assist youth and families in the event of an emergency

· Provides transportation for youth and families throughout the DYS programming continuum.

· Assists in ensuring adequate on-site security; provides routine coverage at the District Office when needed to assist in properly supervising youth while on site – including conducting searches of youth upon entry. Maintains a youth and visitor sign-in log as needed.

· Responsible for the identification and reporting of any on-site physical plant issues or program vehicle maintenance to the Provider Contract Manager.

· Acts as the designated on-site District Office Fire Marshall

· Meets the GPS and/or electronic monitoring needs of each District Office as determined by the region.

· Assists youth in obtaining important personal documentation (Mass Health, Mass ID, birth certificate, etc.)

· Attends weekly team meetings at the District Office to identify and coordinate youth service needs in collaboration with the DYS case management team. The YSC will regularly inform the case management team of new/upcoming resources, information, activities, etc. and the procedures/protocols to access resources for youth and families. This may involve directly providing or arranging for trainings and briefings to case management team members on identified services and resources.

· Maintains documentation of information specific to established community resources and document youth participation.

· Manages on-site funds for client allowances and activity costs. Responsible for the secure storage of rewards and incentives (e.g. gift card) and necessities (e.g. bus passes) to be immediately available on site to serve the needs of clients. Reports to the PCM when responsible for documenting requests, receipt and distribution of funds.

Job Types: Full-time, Contract

Pay: $53,000.00 per year
