Job description
Under the direction of the Director of Family and Community Health Services, the CDCES, will plan, develop implement, and coordinate nutrition programs for the Health Center patients, groups, and community nutrition education programs.
The CDCES will integrate nutritional education into all aspects of primary care, specialty clinics and with external agencies including, but not limited, to the Women and Infants and Children (WIC) Program, and other nutrition-based programs.
50% Direct Patient Care
Provides nutrition assessments for individual patients, prepares nutrition care plans, and educates and motivates patients to accept and follow programs.
Conducts nutrition programs through community events, including nutrition assessments, counseling, education and referrals as requested.
Performs clinical and physical assessments.
Plan, develop, implement and coordinate nutrition programs for health center patients, groups and community nutrition education programs.
Pediatric referrals will include but are not limited to addressing issues of obesity.
Primary Care referrals will include but are not limited to : all patients with diabetes, hypertension, obesity, elevated cholesterol and other nutrition-related complications.
Develops educational plan that takes into consideration individualized nutrition status, educational level, readiness to learn and barriers.
Reviews samples of the instructional material on an annual basis to ensure they support the DSME program curriculum.
Determines and implements appropriate nutrition prescription and / or food plan.
Communicates with practitioners and makes recommendations for outcomes of MNT and self-management education.
Provides education and counseling on an individual basis or in a group setting.
Assists with data collection to determine the clinical and behavioral outcomes of the DSME program.
Adheres to HIPPA guidelines regarding confidentiality of medical information.
Performs other related duties as assigned.
20% Health Education and Outreach
Develops culturally appropriate education materials regarding nutritional issues for use with individual Health Center and SBHC patients and community education programs.
Maintains a library of culturally appropriate educational pamphlets on dietary / food exchanges, purchasing, preparation, and "how to read and understand food labels" to all clients referred to the nutritionist.
Integrates nutrition education into all aspects of primary care, specialty clinics and external agencies, such as WIC.
Develops nutrition interventions to address obesity in patients diagnosed with cardiovascular diseases, including high blood pressure and chronic heart disease, obesity, diabetes Type2, respiratory problems, osteoporosis and cancer.
Participate in internal and external Obesity Studies.
25% Administrative Duties
Develops nutrition policies and protocols based on government guidelines and recommendations for a variety of programs.
Creates and distributes a list of conveniently located health smart markets, exercise programs and nutritional counseling centers to all health center patients.
Evaluates and recommends nutrition education for the Health Center, as well as, other community groups.
Conducts research to obtain a baseline of the nutritional health needs of the MCHC population.
Develops and maintains a nutrition patient tracking / follow-up system.
Partnership with the planning and implementation of nutrition educational programs for patients and staff.
Documents in patient's medical record all nutrition care provided and ensure that appropriate forms are properly completed for billing and productivity reporting purposes.
Participates in a relevant Medical Record review to obtain obesity information.
Participates in external meetings as assigned.
5% Provision of Continuing Education / In-service
Maintains and expands current level of professional knowledge of nutrition theory and practice through continuing education programs / In-services.
Precepts nutrition students, as assigned.
Participates in individual or group teaching sessions.
Participates, as needed on Health Center Committees.
Performs other related duties as required or assigned by the Clinical Health Services Director.
Requirements : Requirements :
The individual must be licensed in the state of Massachusetts as a Licensed Dietitian in Nutrition (LDN), with a bachelor's degree, M.
Ed. or M.S. Degree in Nutrition / Dietetics is required.
At least 3-5 years of experience working in a community health care setting.is preferred. Working in communities with diverse populations in the areas of Pediatrics, Prenatal and Adult Medicine is preferred.
The ideal candidate must possess excellent organizational skills, and Basic Computer operations skills such as Word, Excel, EPIC - OCHIN, as well as demonstrated oral and written communication skills, flexible, organized, and culturally / ethnically sensitive.
Bilingual in Haitian Creole is a plus.
