The Community Development Center, a non-profit agency serving people with ID/DD in the Maury, Giles, and surrounding counties, seeks qualified candidate for the position of Independent Support Coordinator .

The Support Coordinator will provide support coordination with case management to individuals with Intellectual and Developmental Disabilities that include the following components:

• · Communication with individual family
• · Support Plan and follow-up
• · Monitoring and evaluation
• · Community inclusion
• · Advocacy
• · Communication with TN DIDD
• · Communication with TennCare

ISCs provide case management services in the TN Strong Families program. This program helps children who have an intellectual or developmental disability(IDD) and are currently in Tennessee state custody by providing flexible funding so families can pay for needed services and supports. ISCs are responsible for making contact with the family monthly and an in person visit every six months.

Bachelor degree in a social field is preferred. Must be computer literate and have excellent communication skills. EOE/Drug Free Workplace/Vet/Disability/E-Verify

Pay: $16.00 - $16.50 per hour
