PURPOSE OF YOUR JOB POSITION:
The primary purpose of your job position is to plan, develop, organize, implement, evaluate, and direct the Social Work Department in accordance with current existing federal, state, and local standards, as well as our established policies and procedures, to assure that the medically related emotional and social needs of the residents are met/maintained on an individual basis. These responsibilities may include combination of admissions, documentation, and service delivery duties.
MAJOR DUTIES AND RESPONSIBILITIES:
Listed below is an outline of the duties and responsibilities that you will be required to perform. As SOCIAL WORKER, you are delegated the authority, responsibility, and accountability to carry out your assigned duties.
Even though your job description is broad, every effort has been made to make this outline as complete as possible. HOWEVER, we must emphasize that you may be required to perform other related duties and activities.
1) Plan, develop, organize, implement, evaluate, and direct the social service programs of this facility.
2) Meet with administration, medical, nursing staff, and other related departments in planning. social services
3) Develop and maintain a good working rapport with interdepartmental personnel and outside community health, welfare, and social agencies to assure that social service programs can be properly maintained to meet the needs of the residents.
4) Keep abreast of current federal and state regulations as well as professional standards and make recommendations on changes in policy and procedures to the corporate consultant.
5) Serve on various committees of the facility as may be appointed by the Administrator.
6) Utilize social work skills in preparation of residents for admission through thorough inquiry and pre-admission conference with the resident and/or family.
7) Compile and dissimilate information between the referral source and facility staff pertinent to the resident's condition and needs to effect admission in a timely manner.
8) Evaluate resident needs on admission through the completion of a social history, assessment, and discharge plan.
9) Implement the individual resident's social work care plan involving resident/family.
REQUIRED
Four-year degree required.
At least 6 months experience in Long-Term Care Facility
