SUMMARY: The primary purpose of your job position is to provide residents of this facility in your nursing unit with nursing and personal care under the direct supervision of the Rehabilitation Coordinator, and to safeguard the health, safety, and welfare of all residents of the facility, in accordance with the facilitys established policies and procedures and applicable laws and regulations, and the directions your supervisors, who include the Administrator, Director of Nursing, Assistant Director of Nursing, Charge Nurse, Rehabilitation Coordinator and other members of the facilitys management to whom such persons report, in order to assure that the highest degree of quality care is maintained at all times.

MAIN DUTIES

  • Support the facilitys philosophy of care and strive to achieve its goals and objectives.
  • Carry out rehabilitation/restorative procedures as ordered by the physician and recommended by the physical/occupational/speech therapist such as the following:
  1. ROM-AROM, AAROM, PROM
  2. Exercises: self-ranging, wheelchair push-ups, standing tolerance, etc.
  3. Ambulation
  4. Positioning: bed, chair, etc.
  5. Transfer training
  6. ADLs
  7. Application of assistive devices and special programs.
  • Keep daily procedure and treatment record.
  • Write meaningful goal oriented monthly summary progress notes in residents individual chart.
  • Promote and encourage daily care of resident by implementing approved rehabilitation/restorative techniques and training programs.
  • Review and update current care plans with representatives of all departments in Care Plan conferences.
  • Check and follow up (every two hours) turning of bedridden residents, their proper positioning and body alignment in order to prevent pressure sores.
  • Encourage maximum functional ambulation of residents:
  1. Independent walking
  2. Walking with a cane
  3. Walking with a walker
  4. Using a wheelchair
  • Teach and encourage the resident self-care and the use of prosthesis in ADL
  • Participate in ongoing staff education, in-service, and on-the-job rehabilitation/restorative training programs as scheduled or as directed.
  • Identify and report to the Rehabilitation Coordinator needed equipment.
  • Manage the rehabilitative/restorative equipment such as special mattresses, wheelchairs, walkers, cushions, restraints, splints, etc.
  • Assure that work/assignment areas are clean and free of hazardous conditions, i.e. spills, excess equipment, etc. and that equipment, tools, supplies, etc. are properly stored while working, as well as before leaving such areas for breaks, meal times, and end of the work day.
  • Assure that established infection control and standard precaution practices are maintained when performing rehabilitation/restorative procedures.
  • Follow established safety precautions when performing tasks and when using equipment and supplies.
  • Assure that equipment is cleaned and prepared for the next shift.
  • Report all equipment malfunctions and breakdowns to the rehabilitation Coordinator as soon as possible and keep the Rehabilitation Coordinator informed of supply needs and equipment needing replacement.
  • Assure that work schedules are followed.
  • Report all hazardous conditions or equipment to the Director of Rehabilitation.
  • Maintain the confidentiality of resident information and honor his/her personal and property rights.
  • Attend rehabilitation department meetings as directed.
  • Turn in Days Off and payroll special request forms, supply sheets, and annual medical examinations in a timely fashion.
  • Follow established fire, disaster, and evacuation policies and procedures.
  • Perform other duties as assigned by the Rehabilitation Coordinator, Director of Nursing and Administrator.

QUALIFICATIONS:

  • Must be a minimum of eighteen (18) years of age.
  • Must be currently certified as a Certified Nurse Aide in accordance with laws of the State of Illinois.
  • Must have current CPR certification.
  • Must have received specified training in a rehabilitation course in accordance with the course outlined and approved by Illinois Department of Public Aid.
  • Must demonstrate the ability to make independent decisions in a timely fashion when circumstances warrant such action.
  • Must demonstrate the ability to deal tactfully with staff, residents, family members, visitors, government agencies and their representatives, outside health care professionals, support agencies, and the general public.
  • Must demonstrate the ability and willingness to work harmoniously with professional and non-professional personnel.
  • Knowledge of JCAHO, OBRA, IDPH, and IDPA regulations.

  • Required to sit, stand, bend and walk regularly; lift and/or move up to 100 pounds.
  • Visual and auditory ability sufficient for written and verbal communication.
  • The noise level in the work environment is usually moderate.
