THIS POSITION IS COMPLETELY REMOTE HOWEVER TO BE CONSIDERED YOU MUST RESIDE IN PACIFIC TIMEZONE TO BE CONSIDERED PPREFERRABLY CALIFORNIA

As a case manager you are responsible for health care management and coordination of members in order to achieve optimal clinical, financial and quality of life outcomes. Works with members to create and implement an integrated collaborative plan of care. Coordinates and monitors member's progress and services to ensure consistent cost effective care that complies with policy and all state and federal regulations and guidelines.

This is a non-negotiable position, and the candidate must be licensed as either an RN or LCSW. The rate for this position is up to $50.00 per hour depending on experience and will require a laptop, keyboard, mouse, headset, and dual monitors.

Requirements:

• Bachelor’s degree in a Nursing or Master degree in Social Work or Health Education
• Active, unrestricted State Registered Nursing license or Licensed Clinical Social Worker LCSW
• UAS certified
• 2 years of clinical experience with case management experience
• Strong technical skills (utilizing multiple programs at the same time) along with competency in Microsoft applications (Outlook, Teams, Excel, etc.)
• This will include potential home visits to complete assessments as well as completion of telephonic assessments
