JOB DESCRIPTION

African Services Committee is the oldest and largest non-profit serving the African immigrant community in New York City. Established in 1981 by Ethiopian refugees, African Services was initially set up to provide comprehensive resettlement assistance to African refugees and asylum seekers.
Today, ASC provides health, housing, legal, educational and self-sufficiency services to African and Caribbean immigrants, refugees and asylum seekers affected by war, poverty, global health disparities, political oppression, and social persecution based on sexual orientation or HIV status. Our programs are designed to meet the basic needs of newcomers, using health as a gateway to immigrant integration and supporting community members to live healthy and productive lives.

This is a temporary position with the possibility of extension or permanent hire.

Basic Functions of Position

The Case Manager (CM) coordinates the medical and supportive care of individuals living with HIV/AIDS. The CM provides a range of client-centered services to link clients with health care, psychosocial, mental health, housing, substance use, and other community services. The CM maintains regular contact with the client regarding medical treatment and all other service referrals to ensure coordinated and timely delivery of services and continuity of care.

Major Duties and Responsibilities

- Conduct client screening and intakes.

- Conduct regular comprehensive and timely assessments and reassessments.

- Arrange supportive services for clients, including assisting with completion of insurance applications, affordable/safe housing, food and transportation assistance, mental health referrals and others.

- Follow-up with all referrals given to ensure services have been provided,

- Develop comprehensive, individualized, client centered service plans.

- Develop a and facilitate client service plan and monitor monthly.

- Monitor monthly to assess the efficacy of the plan.

- Work collaboratively with all members of the client service plan team including medical, mental health, substance use and community service providers.

- Document activities through case notes, assessments, and other agency forms.

- Advocate for appropriate services for clients based on needs and assessments.

- Foster client independence through education, supportive services, and other methods of empowerment and efficiency building.

- Conduct visits with clients in various settings, including in the community and client’s home and other appointments, where appropriate.

- Develop community relations to foster collaborative outreach to targeted communities.

- Attend professional conferences, summits, meetings, and training as required.

- Help identify and participate in appropriate agency outreach events.

- Participate in regular agency meetings including weekly supervision meetings.

Qualifications

1. Bachelor’s Degree (B.A. or BSW) and a minimum of two years’ case management

experience and/or equivalent.

2. Minimum of one year’s experience providing HIV/AIDS service and/or equivalent.

3. Ability to work effectively with diverse gender identities, races, ethnicities, ages, sexual orientations, and drug use patterns, in a multicultural environment.

4. Must be able to work independently as well as in a team environment.

5. Must demonstrate sound judgment, initiative, and discretionary abilities.

6. Must be bilingual in English/French. Fluency in one or more major African languages is a plus.

7. Basic computer skills a must.

5. Problem Solver- capable of thinking on the spot. Able to assess relevant information, identify a range of options, and make recommendations to resolve the situation.
