Responsibilities:

• On-call hospice visits afterhours with an opportunity for daytime visits
• Provide accurate and timely documentation
• Assess and provide patient and family/caregiver education
• Perform other duties as assigned by the registered nurse

• Other visit as needed based on your availability including routine and PRN, visits, discharges, GIP, CC, etc.

Qualifications:

• Compassionate and caring demeanor
• Currently licensed as an LVN in CA
• Valid CA Driver License
• Hospice care experience, understands philosophy of hospice concept, and understands needs of the terminally ill
• Previous experience in nursing or other medical fields

Job Types: Part-time, PRN, Per diem

Pay: $30.00 - $40.00 per hour
