- Assists patients in identifying community resources available to meet their needs
- Home visits may be conducted depending on the needs of the patient
- Assists patients in navigating the healthcare system by scheduling appointments, coordinating referrals to specialists, and managing insurance and billing issues
- Advocates for and links patients to community services and assists in assessing available support services
- Keeps information on partner organizations updated, by making routine contact with to verify services and eligibility requirements

- Excellent communication and interpersonal skills, with the ability to build rapport and trust with patients and their families
