Growing Home Southeast is Hiring a FRCPS Case Manager Near Camden, SC

Hiring statewide for Family Resource Connection and Preservation Services Case Managers.
The primary responsibility of a FRCPS Case manager is to provide assessment, support and case management services to children and families referred by the South Carolina Department of Social Services (SCDSS) who have an open Family Preservation Case, open Investigation, or are in a licensed kinship placement or unlicensed court-ordered kinship placement.
While maintaining the confidential nature of the agency’s services, Case Manager duties include but are not limited to the following: assessment, service planning, monitoring, home visitation, counseling, parent education, concrete service provision, and community referrals. Services are based on family needs.
Principal Responsibilities:
  • stablish a professional relationship with families and be a consistent role model to those served.
  • Establish and conduct the initial family meeting in accordance with service standards.
  • Assess the needs of families and engage families in service planning. Monitor the family service plan making updates and noting progress as necessary in accordance with program requirements and agency expectations.
  • Maintain a caseload in accordance with program standards and agency expectations and meet with designated families for the prescribed number of hours/days/contacts per week.
  • Submit accurate and complete service documentation within 48 hours of providing service (or as required by program and agency expectations) and submit required reports within assigned timeframes.
  • Plan and implement child development, parent education, case management, parenting interventions, and family support services through home visits and phone calls as appropriate and indicated in service plans, program requirements, and agency expectations.
  • Refer and link families to community-based programs and services to strengthen the family unit, including facilitating access to specific concrete goods.
  • Actively participate in required supervision, in-service training, and staff meetings as required and attend various internal and external meetings to ensure appropriate program implementation and high-quality supports for families.
  • Coordinate activities for families to include family literacy, social and educational events, and facilitate parent involvement in community activities (activities may include socializations, parent meetings, and parent trainings)
  • Maintain open communication with referring authorities, agency leadership and team members to ensure the most appropriate services for families.
  • Transport youth and families in personal vehicles as necessary
  • Attend court hearings and provide testimony as necessary and required.
  • Ensure contract compliance regulations are being followed properly.
  • Other duties as assigned.
Knowledge, Skill and Experience Required:
FRCPS case manager must have at least a bachelor’s degree in the field of Psychology, Social Work, Marriage and Family Therapy, behavioral science, or a related field, and a minimum of one year of direct care experience or supervised practice in lieu of the experience requirement.
Must possess warm, empathic relationship-building skills and communication style to establish a trusting relationship within a family. Must demonstrate knowledge, understanding, and experience in basic parenting skills and family dynamics, behavior management, parent education and childcare/safety. Must serve as a positive role model to families and be sensitive to the needs of youth and families in crisis. Must be aware of, sensitive to, and knowledgeable about various cultural, intellectual, economic, social, spiritual, and gender-related differences.
Must have adequate transportation with appropriate insurance coverage for transporting children and other family members as required by Growing Home Southeast Policy. Must have the ability to drive for long periods of time, sit, stand, and lift up to 10 pounds. Must demonstrate ability to work effectively as a member of a team and exhibit organizational and writing ability to ensure accurate and complete records and required documentation.
