Part Time Time Day 8am - 4:30pm The Community Health Nurse is responsible for the healthcare needs of the Independent Living Community. This role is also responsible for making critical decisions regarding the healthcare of the residents to maintain the highest level of care with the best possible quality of life. EDUCATION AND EXPERIENCE REQUIREMENTS:
Current state professional nursing license (RN) or practical nursing license (LPN) required
Two years' experience, geriatric/long term care experience preferred
Active CPR certification required
Ability to instruct, direct and lead team members required
Demonstrated ability to evaluate the health status of a resident followed by a plan of action consistent with quality nursing practices required
EXPECTATIONS AND RESPONSIBLITIES:
Regularly conducts home visits with residents for evaluation and data gathering of health care status
Develops, initiates and alters an interdisciplinary plan of action in consultation with physician, Health Center Administrator, Director of Nursing, Director of Admissions and Executive Director
Implements and maintains nursing policies, procedures, objectives and standards of practice for Community Health
Conducts nursing outreach programs on a regular basis
Assists with the administration of medications and treatments in compliance with federal, state, and local laws, and with the community policies and procedures
Actively participates in all required in-service programs and team member meetings, including program presentation
Organizes and conducts resident programs
Actively participates in the community Quality Assurance/Quality Improvement Program
Uses standard precautions to prevent spread of communicable diseases, and enforces and trains proper infection control practices to team members, residents and visitors
