Job description

Physical Therapist

Visiting Nurse & Community Care is a leading, not-for-profit home health agency. We provide employees excellent opportunities for growth and development in a rewarding team-oriented work environment.

Responsibilities:

  • Develop and implement physical therapy plans of care ensuring quality and safe delivery of physical therapy home health care services.
  • Work directly with patients to achieve maximum physical recovery.
  • Evaluate effects of therapy treatment and communicate patients' progress.
  • Record and document patient care services.
  • Promote education for clients, client families and the community.
  • Coordinate provision of physical therapy home health care services that reflect VNCC’s philosophy and standards of home health for assigned clients

Qualifications:

  • Current PT licensure in MA required.
  • Two (2) years experience in home health care, elder services, and/or public health preferred.
  • Strong assessment skills needed.
  • Excellent communication skills with a customer service orientation necessary.
  • Ability to build rapport with patients.
  • Ability to handle physical workload.
  • Work with a friendly and supportive team of seasoned clinicians at an agency that understands the need for work/life balance.
