POSITION SUMMARY:

Founded in Oakland, California, the mission of Roots Community Health Center is to uplift those impacted by systemic inequities and poverty. We accomplish this through medical and behavioral health care, health navigation, workforce enterprises, housing, outreach, and advocacy.

Under the supervision of the Medical Director, the Primary Care Physician will provide patients with high-quality treatment and diagnostic services. The ideal candidate is an intuitive healthcare professional whose diagnostic abilities are only matched by their concern for their patients. This position is responsible for meeting, assessing, and diagnosing patients in our clinics and providing them with healthcare services that meet state and national standards for caregiving. They will be responsible for prescribing patients with appropriate medication, health regimens and lifestyle adjustments that will contribute to their recovery from diagnosed health issues, prevent future health issues and promote a healthier life. The right candidate will have a passion for providing diagnostic services to patients and will enjoy working closely with other healthcare professionals in our organization.

DUTIES AND RESPONSIBILITIES:

  • Provide leadership, direction, oversight and training as needed for clinical team.
  • Prescribe or administer treatment, therapy, medication, vaccination, and other specialized medical care to treat or prevent illness, disease, or injury, according to the highest standard of care.
  • Order, perform, and interpret tests and analyze records, reports, and examination information to diagnose patients’ condition(s).
  • Provide culturally responsive and situationally appropriate services for predominantly African/African American patients and families.
  • Encourage patient self-empowerment, building skills of self-advocacy and self-efficacy.
  • Collect, record, and maintain patient information, such as medical history, reports, and examination results.
  • Monitor patients' conditions and progress and reevaluate treatments as necessary.
  • Explain procedures and discuss test results or prescribed treatments with patients.
  • Refer patients to medical specialists, wraparound services, and other practitioners when necessary.
  • Advise patients and community members concerning nutrition, lifestyle, coping skills and risk reduction.
  • Coordinate with nurses, social workers, pharmacists, psychologists, and other health care providers.
  • Direct and coordinate activities of clinical staff, students/externs and support staff.
  • Perform duties and procedures within scope of clinical license and scope of primary care.
  • Plan, implement, and administer health programs or standards in clinic and the community for prevention or treatment of injury or illness.
  • Meet performance targets set by the organization for selecting treatment protocols.
  • Provide patient education and support as needed.
  • Comply with all credentialing requirements in a timely fashion.
  • Attend all required department and organization meetings and training, as assigned.

COMPETENCIES:

  • Licensed M.D. or D.O. from a recognized University.
  • 2 to 5 years’ experience working in community health or primary care clinic setting preferred.
  • Culturally competent working with African American patients and families.
  • Current valid California medical license and Drug Enforcement Administration (DEA) registration in good standing.
  • Successfully completed residency in Family Practice, Internal Medicine or Emergency Medicine.
  • Excellent leadership and communication skills.
  • Board certification in Family Medicine ( /-Adolescent Medicine) or Internal Medicine preferred. Board eligible acceptable.
  • Experience working with Electronic Health Records.
  • Ability to keep accurate, clear and legible records.
  • Ability to maintain confidentiality and impartiality.
  • Compassion and a good bedside manner.
  • Ability to solve problems and make and communicate decisions effectively.
  • Ability to interact effectively and in a supportive manner with persons of all backgrounds.
  • Willing to train on health coaching, Hepatitis C treatment and other beneficial/emerging topics.
  • Proficiency in working with an urban underserved population.
  • Experience in working with individuals recently released from incarceration, homeless and other marginalized populations.
