Position Summary

Under the supervision of the Transitional Housing Manager, The Assistant Case Manager Coordinator (ACMC) will be responsible for daily management and operations, direct oversight of the Assistant Case Managers (ACM) and the resident community. The ACMC will provide administrative/direct program supervision to the program staff to ensure all services are delivered according to local, state, and federal laws and in compliance with contractual requirements. The ACMC will be required to produce schedules, reporting on s, performance measures, and be heavily involved in Veteran social activities. The Assistant Case Manager Coordinator is more than just a friendly presence; they are the crucial link in creating an environment in which our Veterans heal, grow and prepare to re-enter society as productive, self-sufficient individuals.

Veterans Place of Washington Boulevard is staffed 24 hours a day 7 days a week. The Assistant Case Manager

Coordinator must be available during all days and shifts. The ACMC will be required to schedule all ACM's, be on-call for any emergent needs, and adjust the schedule should an ACM call off.

Position Responsibilities

Supervision, Training, and Accountability:

  • Provide leadership and direct supervision to all program and outreach specialists. This will be done through reviews, consultation, training, performance evaluations, identifying and resolving performance issues, and support to ensure compliance and program quality according to the organization's policies and procedure, CARF, and best practices implementation
  • Communicates important and arising issues to Transitional Housing Manager and, where needed, participates in development of an appropriate course of action.
  • Evaluates individual and overall team performance on a regular and ongoing basis, using collected data and information to make needed improvements to maximize quality of care and staff productivity
  • Ensures that participant and program goals (program, financial, personnel, professional development, and trainings) are met in accordance with established program policies and procedures. Participates in the development of program and program performance according to program guidelines.
  • Conduct performance evaluations for the ACM team; proactively identify and resolve performance issues. Monitor performance goals and professional development in accordance with agency expectations
  • Ensure supervisees' timesheets, expenses, leave requests, etc. are complete, accurate, and in compliance with agency policies, procedures, and practices.
  • Participate in professional development networks and associations to stay abreast of emerging trends and best practices.

Service Activities:

  • Oversee and provide transportation for Transitional Housing Veterans
  • Weekly reporting and report documentation to supervisor
  • Inspections and oversight of inspections
  • Assesses needs and interests of Veterans Place residents by interacting with them on a regular basis
  • Initiates programs, activities and community development that address the social, recreational, educational and development aspects of the residents' lives and promote positive relationships, sobriety, and supportive community engagement
  • Assists with orientation and welcoming of new residents
  • Works with staff members to promote responsible living and a culture of sobriety at Veterans Place
  • Assists in collection and delivery of drug screen samples to the Veterans Administration for processing
  • Monitors physical facilities, conducts building rounds as needed and performs light maintenance and grounds tasks (e.g. switching blown breakers, spreading salt on iced walkways, etc.)
  • Maintaining the physical inventory of Transitional Housing supplies and the purchasing of supplies
  • Oversight of Finger Scanner requirement for Veterans reporting as on-site

Clinical Activities:

  • Maintain case records and other documentation in accordance with program guidelines and professional requirements. This includes shift reports, group attendance reports, DAU reports, inspection reports, etc.
  • Exercise and coordinate a variety of individual and group counseling techniques that are evidence-based and demonstrate good clinical practice. Implement legal and ethical standards of service delivery, privacy, and confidentiality, complying with all applicable regulations.

Program Development:

  • Be active in Veterans Place social activities for residents
  • Develop goal plans with staff to develop organization strategies
  • Oversee the delivery of services within the scope of this program and make recommendations to Transitional Housing Manager

Communication:

  • Prepare reports as requested by supervisor
  • Participate in administrative, staff, clinical, and in-service activities and training as required.
  • Communicate policies and the rationale behind them to veterans and staff
  • Inform other agency staff members of relevant matters related to the veterans
