Neighborhood Health – Pharmacist PRN

Our goal is simple: improving access to healthcare in our community and surrounding areas. Due to expansion, Neighborhood Health is looking to add a Pharmacist PRN to our team. Together, we are all committed to building healthier communities by delivering comprehensive, quality health care with compassion and respect.

As a Pharmacist PRN, you will:

  • Conducts and evaluates medication histories, assesses compliance, and suggests modifications for optimal drug product selection in order to achieve desired outcomes; instructs patients on correct use of prescribed medications. 
  • Instructs patients in correct use of prescribed medications and identify any barriers to patient compliance with drug therapy.
  • Effectively verifies patient medication orders for appropriateness and accuracy. Provides medication reconciliation and evaluates medication appropriateness and completeness with respect to patient data, including age, height, weight, diagnosis, and other pertinent patient information.
  • Prevents, detects, and resolves medication-related problems. Makes evidence-based, patient-centered drug therapy recommendations as part of the Integrated Care Team in a PCMH Model.
  • Provides information, education, and counseling to patients about medication-related care.

Requirements:

  •  At least one (1) year of pharmacy experience preferred
  • 340B experience preferred. 

Education Requirements:

  • Doctor of Pharmacy (PharmD) degree preferred or Bachelor’s degree in Pharmacy.
