Oregon Healthcare & Rehabilitation Center is a small 45 bed long term/skilled care facility located in beautiful Oregon Wisconsin. General Purpose:

Manage administrative and funtional areas or programs within the nursing department. Assist the Director of Nursing in the overall operations of the department. Conduct and coordinate the development and completion of the resident assessment process in accordance with the requirements of the Federal and State regulations as well as Company policy and procedure.

Brief Description of Job Duties:

• Oversee and coordinate the development and completion of the resident assessment (MOS) in accordance with current Federal and State rules, regulations, and guidelines that govern the resident assessment, including the implementation of RAPs and Triggers.

• Assemble information from the Initial Nursing Assessment, resident interview, and clinical record review to complete the nursing portion of the Minimum Data Set within 10-14 days of admission or annual review, and when there is a significant change in a resident's condition.

• Notify all members of the interdisciplinary team at least one week in advance of the MOS due date for all new admissions, annual reviews, and significant changes in resident condition.

• Monitor and follow-up with team members as needed to verify that all assigned sections of the MOS are completed, dated, and signed within designated time frames.

• Assist the DON in planning, developing, organizing, implementing, evaluating, and directing the day-to-day functions of the nursing department

• Make written and oral reports/recommendations to the DON as necessary/required, concerning the operation of the nursing department

• Confirm all nursing personell are following their respective job descriptsions

•In the absence of the DON, serve on, participate in, and attend various committees of the facility

• Assist with interviewing, orientating, training, evaulating, and supervising the nursing department

Minimum Qualifications:

• RN/LPN with current state license

• Organized and detailed work performance

•Good communication skills with excellent self-discipline and patience

• Genuine care for and interest in elderly and disabled people in a nursing facility

• Perform all other essential duties as needed
