This facility serves anyone residing in Larimer County in need of urgent intervention. This will be accomplished through effective assessment, care management and collaboration within the scope of practice and under the supervision of nursing staff and under the orders of a medical provider. This position requires knowledge of diversity and inclusion with the relevant ability to engage, communicate, interact, and work effectively and cooperatively with people of different backgrounds, identities, and culturally diverse communities.Duties Include:• Checking in all clients, including all vital signs.• Provide medical support for on-site and telehealth providers.• Attending and participating in daily huddles to discuss client care.• Refilling client’s medications when requested by phone or fax, if request is within established protocols.• Obtaining prior authorizations when needed for client’s medication coverage.• Triaging and returning client phone calls/voice messages, addressing requests and/or concerns.• Document all pertinent phone calls, including client concerns and issues.• Obtaining or ensuring required releases of information are in clients record prior to speaking with or answering any inquiries regarding that client.• Assisting with communication between SHP providers and their clients, pharmacies, SHP clinicians, outside medical providers, and schools via phone, email, and fax.• Procuring external medical records, medication list, lab reports, and/or discharge reports when requested by provider, ensuring the necessary ROI is current and in client’s record.• Order and receive lab work when directed by providers, entering lab results in clients records and informing providers when labs are out of normal range based an established protocol.• Reviewing providers schedules and preparing their upcoming appointments daily ensuring all information needed for encounter is obtained prior to client arrival.• Communicate with medical schedulers to coordinate clients and provider’s needs.• Must have excellent phone, computer, and interpersonal skills, promoting cooperation within the team and the highest level of care for our clients.• Flexibility with processes changes and shifts within duties or location assigned, when indicated to meet our clients’ needs and/or our team’s needs.• Willingness to train in and provide coverage at other clinical sites as neededRequired:Valid Colorado driver’s license, graduation from a medical assisting programPreferred:2 years in a behavioral health setting, certification in medical assistingBilingual/bi-cultural preferred.Start With YesAt SummitStone, we show up not only for our clients, but for our staff as well. Employees working at locations operating 24/7 earn a differential. Shift differentials are also available for evenings, nights, weekend and holidays.
