UCP Heartland, Gibbs Center, Jefferson City is looking for an LPN (Licensed Practical Nurse), who is ready to provide direct services to our amazing clients!

Do you have:

Your LPN in Nursing from an accredited college, university or hospital
Required licensure and registration with the state of Missouri.
One Year of experience working with people with developmental disabilities
Expertise and familiarity with electronic records systems as well as basic computer software
Excellent written and verbal communication skills
Ability to transfer adults who may weigh in excess of 150 pounds using mechanical supports.

So, what does the job look like?

Ensure the physician orders are present, current and inclusive of all required information per Agency and DMH policies.
Administer medications and/or treatments as prescribed (g-tube feedings, nebulizer treatments, etc.) for all Clients ensuring prompt, thorough documentation in accordance with Agency policy and accepted standards of care.
Assures medication storage and disposal in accordance with Agency requirements.
Provide care for Clients in need of first aid or more advanced medical services or who are otherwise ill.
Daily charting of medications and Client health information using an electronic records system.
As needed, provide for and assure that personal care needs (ADLs) of Clients are met.
Facilitate and monitor Clients’ special diet orders.
Assure compliance with Agency policy regarding medical protocols (annual physicals and updated medical records) for Clients in coordination with the RN and Agency’s Medical Director.
Teach level 1 Medication Administration Training for Direct Support Professionals and serve as a coach/consultant regarding medication errors.
Interact on a professional level with families and physicians to assure that Client needs are presented and addressed.
Provide staff training in the areas of health management, medication administration, and infectious disease control among others.
Acts in the absence of the Registered Nurse.
