Now hiring Caregivers / Home Health Aides (HHA) focused on providing care to clients to help them maintain their health and independence. This position will be focused on providing care in the DULUTH area.

**Position duties include**

* **Visiting clients in their homes and helping them maintain their independence.** There really is no place like home! You'll be helping families in your community stay in their homes

* **Provide care according to the client's Care Plan.** This can include recording vitals, grooming, bathing, and assisting with other treatments.

* **Building relationships with your clients**, and reporting any health concerns to the Nurse Case Manager
