The medical director of the population health clinic serves as the primary liaison to the providers, practices, and care sites supporting the Doctors Hospital population health clinic to ensure that every patient receives the care they need, when they need it. The medical director works collaboratively with other clinic medical directors and faculty, as well as the inpatient teaching services and other GME programs, to ensure patients receive timely care, appropriate resources, and establish with a primary care clinic and/or specialty clinic. The medical director will collaborate with physicians and hospital leadership to create and execute strategies that will best serve the patient population and align with goals of the care site, and drive strategies for success around walk in care, transition of care and other comprehensive care. This individual will be responsible for clinic operations and daily management and will inform additional opportunities in population health.
