Job Details

• Conducting home care visits for adult and geriatric patients
• Managing OASIS admissions, recertifications, and creating 485s
• Coordinating care with interdisciplinary team members
• Providing skilled care such as wound care and medication management
• Maintaining open cases and ensuring proper documentation

Skills You Need:

• RN licensure
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Willingness to learn and adapt to new challenges
