West Bloomfield Health & Rehabilitation Center is currently looking for a full-time Nurse Reviewer (RN or LPN) that would be available to work weekdays, and (2) weekends per month (rotating basis). Candidates who are unable to work any Saturdays & Sundays need not apply. Only LPN's with existing SNF or Hospital experience will be considered.

Job Description:

Tired of working the floor and providing direct patient care? Our Nurse Reviewers utilize their clinical knowledge and nursing experience, by doing desk reviews of hospital referrals for inpatient placement at our skilled nursing facility. The nurse will use his/her professional judgment to evaluate the patient's information to ensure that appropriate services are able to be provided at our facility.

Additional Duties:

• Act as a liaison between the facility and the outside community
• Connect daily with referring hospitals and case managers
• Conduct admission screening of potential patients; determine level of care, services required, equipment needs and insurance coverage, as indicated

Education/Experience:

• Registered Nurse graduated from an accredited program
• Current unrestricted Registered Nurse (Michigan license)
• LPN currently working towards RN
• Experience with Allscripts, EPIC, Careport, and EMR's is essential
• Valid Driver’s License and reliable automobile transportation for on-site assignments and off-site work-related activities
• 3 or more years' experience in a related clinical setting
