Responsibilities

The Palliative Care Social Worker (PCSW) Provides intensive psychosocial therapeutic support services to palliative care patients and families to help meet social, emotional and financial needs related to the impact of a chronic or terminal illness, grief, and loss.

Qualifications

1. Licensed Clinical Social Worker LCSW2. Advanced Certified Hospice & Palliative Social Worker ACHP-SW certification or ability to take exam within 24 months of being hired.3. Basic Life Support BLS through the American Heart Association Preferred: 1. Minimum of two 2 years working in a direct clinical therapeutic capacity2. Minimum of two 2 years working with life-threating conditions, chronic disease, palliative care and or hospice, end-of-life care, and related situations3. Knowledgeable of a full range of specialized and expert clinical social work counseling services for patients families receiving palliative care4. Able to provide crisis intervention, short-term and family therapy; Offers information and referral and or tangible assistance5. Organizational and prioritization skills while determining severity of patient condition and prognosis6. Excellent verbal and written communication skills7. Basic computer skills
