United Methodist Communities in Camden County, a 5-Star full-service senior living community that is a model in person-centered care community, located in beautiful Collingswood, NJ is seeking a Healthcare Lead Social Worker .


Requirements for a Lead Social Worker:

  • Bachelor's degree in Social Work (BSW) or Human Service Field
  • Bachelor's degree in Social Work (BSW), and Certified Social Worker license with the State of New Jersey (CSW) perferred
  • Proficiency in a clinical software system.
  • Understanding of the MDS, care plan and admission and discharge processes.
  • One year Healthcare/Long Term Care experience.
    Proficiency in multiple clinical software systems preferred
  • Five years' experience in the Healthcare/Long Term Care setting preferred
  • Certified Social Worker license with the State of New Jersey (CSW).
  • ACSW preferred

Responsibilities for a Lead Social Worker:

  • Develop, maintain and utilize a listing of current community resources.
  • Advocate for the resident and their family.
  • Understands and meets all requirements for social service documentation, i.e. interdisciplinary care plan; d/c notes; advanced
    directives; resident rights, etc.
  • Prevents and addresses resident abuse.
  • Educate associates regarding residents' rights including how to recognize and prevent abuse, neglect and mistreatment, resident
    dignity, independence and quality of life - as outlined in the Resident Rights.
  • Provide the residents, families, significant others and associates with communications regarding care conferences such as the date
    and the time of the meeting.
