Summary of Roles & Responsibilities:

· Work with medical assistant to contact patients for follow up care and documents accordingly

· Perform timely documentation in medical records

· Perform comprehensive evaluations including history and physical exams

· Procedure may include (PUR SCAN, Hormone replacement therapy, pain management, women’s health and men’s health, etc.)

· Provide consultations if necessary

· Assist in training with staffs and attend weekly meeting

· Prescribe medications and administer treatment as needed

· Collaborate with other healthcare professionals to develop and implement patient care plans

· Educate patients and their families on preventive care, disease management and healthy lifestyle choices

· Maintain accurate and up to date medical records in compliance with HIPAA regulations

Qualifications for nurse practitioner:

· Current certification through either American Academy of Nurse Practitioner or American Nurses Credentialing Center

· Current active license in Texas

· NPI/ DEA prescriptive authority

· BCLS
