Title: HIV Peer NavigatorReportingTo: Nurse Care ManagerSchedule: 15 hours per weekRate: $18.38/hrPosition Overview:The HIV Peer Navigator plays a critical role in providing support, guidance, and advocacy for individuals living with HIV/AIDS. As a trusted peer, the Navigator will work closely with clients to help them navigate the healthcare system, access services, and improve their overall well-being. This position requires empathy, understanding, and the ability to effectively communicate and connect with individuals impacted by HIV/AIDS. This position includes no managerial or supervisory responsibilities.Essential Duties & Responsibilities: Escort patients to appointments.Work with a multidisciplinary team of medical providers, medical case manager, community health worker and prevention staff to identify HIV persons who are not in care and work to re-engage those patients in care.Responsible for providing portal introduction and education.Will assist in conducting active outreach to make contact with and engage identified patients. Outreach includes, but is not limited to health events, phone calls, letters, text messages, home visits, etc.Will provide transportation assistance to patient’s as scheduled.Will help to provide specific education for disease self-management.Provides continuous treatment adherence support for patients.Assists in identifying and navigation of any barriers to care and adherence.Provides emotional support and counseling as needed.Provides various types of support and education including healthy lifestyle recommendations, support for family members, and end of life support/assistance.Assist with the establishment of referral sources in the medical community.Qualifications:Must have a high school diploma or equivalency.Applicant must be a peer – that is, represent the population to be served by the particular program in which they will be working in.Must have highly developed verbal and written communication skills and the ability to work effectively with administrative, clinical and support staff to accomplish the goals of the program.HIV/AIDS/STI knowledge including prevention, infection, and treatment.Demonstrated interpersonal skills and ability to engage and work effectively with diverse staff and patient population is required.Ability to maintain confidentiality and adhere to ethical guidelines.Empathy, compassion, and a non-judgmental attitude.Must be able to work independently and as a part of a team.Physical Demands & Working Environment: Most work is completed in the community, with occasional work completed in office.Not substantially exposed to adverse environmental conditions (Typical of most clerical and administrative positions).Sedentary workequivalent to lifting approximately 10 pounds with occasional lifting and/or carrying of small objectsFrequent and extended walking and standing. Seldom bending, climbing, reaching and lifting.Follows clearly defined policies and procedures.Makes decisions within well-defined parameters for the completion of own tasks only.Supervision Received:Routine supervision:Provided with instructions on job requirements, methods, priorities and deadlines with particular attention to unusual or difficult aspects or tasks.
