Hospice Physician - part-time, 8 hours per week

Functions & Responsibilities:

· Acts as a team physician for the community interdisciplinary team, coordinating and collaborating on responsibility for hospice care with the hospice Medical Director

· Reviews clinical material of referring sources, providers, and hospice interdisciplinary team to certify/recertify that the patient’s prognosis and goals of care are appropriate for hospice services.

· Makes patient visits as indicated by clinical circumstances if needed, for patients with symptom management needs, no other attending and hospice Face-2-Face visits.

· Collaborates with the patient’s attending provider and, as needed, encourages the patient’s attending provider to provide primary care to his/her patient in collaboration with the interdisciplinary team.

· Participates in the development, review and modification of comprehensive plans of care with other members of the interdisciplinary team.

· Reviews the medical care provided in patients’ home and in inpatient health care facilities.

· Performs palliative care consultations as requested.

· Performs other related duties of a similar nature and complexity as directed.

Education: M.D. or D.O., licensed to practice in MA, Hospice and Palliative Care certification preferred.
Experience: Minimum 3 years of medical practice. Experience in hospice and/or palliative care.
