Responsibilities of an MDS Coordinator:

• Conduct and coordinate the Minimum Data Set (MDS) assessments accurately and timely for all residents.
• Collaborate with interdisciplinary teams to gather information and develop comprehensive care plans.
• Ensure compliance with federal and state regulations related to MDS assessments and care planning.
• Participate in the development and implementation of policies and procedures related to resident assessments.
• Act as a liaison between the nursing staff, physicians, and other healthcare professionals to communicate resident needs and changes in condition.

Qualifications of an MDS Coordinator:

• LPN licensed in PA
• Responsible, ready, and eager to work
