Advanced Home Health and Hospice is looking for a MSW to join our hospice team on a part time basis with growth into full time. We are looking for an MSW to support our Fernley and Fallon, NV patients. This MSW will help manage our patients Monday to Friday with weekends off. The person will also help collaborate with our highly skilled team including RN's, hospice aides, MD's and our chaplain.

Responsibilities:

• Meet with patients per request regarding a range of social services needs.
• Assess nature of clients' situations by interviewing and reviewing personal history
• Determine appropriate milestones to gauge client progress
• Communicate client updates to all relevant parties
• Facilitate referrals to other healthcare professionals and programs
• Maintain accurate client documentation

Qualifications:

• Current State Social Workers License: MSW
• Previous experience in social work, counseling, or other related fields
• Compassionate and caring demeanor
• Ability to build rapport with clients
• Strong leadership qualities
• Excellent written and verbal communication skills

Job Type: Part-time

Pay: $85.00 - $100.00 per hour
