Salary Range: $19.00 - $19.00

Description

JOB SUMMARY:

A person who is specifically trained, competent and performs assigned functions of household chores, assistance in completing activities (shopping & appointments outside the home), companionship, assistance with personal care and activities of daily living to the patient in their residence under the direction, instruction and supervision of the a designated Supervisor.

RESPONSIBILITIES

  • Understands and adheres to established Agency policies and procedures.
  • Maintains communication with the supervisor when conditions are unsafe or goals cannot be met.
  • Maintains a safe environment while performing job assignment.
  • Performs personal care and activities of daily living as spelled out in the service plan.
  • Completes appropriate visit records in a timely manner as per Agency policy.
  • Reports changes in the patient's condition and needs to the designated supervisor and/or Director of clinical Services.
  • Assists with activities of daily living, personal care (as outlined in Chapter 26 of licensing) and any other assignments as included in the service plan.
  • Provides companionship including, but not limited to, social interaction, and conversation.
  • Reminds consumer to perform ordered exercise program.
  • Assists with feeding only as long as consumer is able to independently chew.
  • Assist with ambulation only if consumer has the ability to balance and bear weight.
  • Assists with the emptying only of urinary collection devices.
  • Assists with the emptying of ostomy bags.
  • Assists with transfer only when the consumer has sufficient balance and strength to reliably stand and pivot and assist with the transfer.
  • Provides accompaniment to medical appointments, banking errands and basic household errands. Cannot take consumer in their vehicle or ride with them in their vehicle.
  • Assists consumer with medication reminding only when medications have been preselected. .
  • Attends in-service and continuing education programs as scheduled and necessary.
  • Attends patient care conferences as scheduled.
  • Performs household services including cooking and meal preparation, cleaning and laundry essential to health care in the home as assigned.

WORKING ENVIRONMENT:

Works indoors in Agency office and patient homes and travels to/from patient homes.

Qualifications


QUALIFICATIONS:

  • Must have completed agency training or have verified experience in the provision of home care tasks to consumers and passed a competency evaluation. 
  • Have a sympathetic attitude toward the care of the sick and elderly.
  • Ability to carry out directions, read and write.
  • Maturity and ability to deal effectively with the demands of the job.

LIFTING REQUIREMENTS:

  • Ability to perform the following tasks if necessary:
  • Ability to participate in physical activity.
  • Ability to work for extended period of time while standing and being involved in physical activity.
  • Heavy lifting up to 75 lbs.
  • Ability to do extensive bending, lifting and standing on a regular basis.
