Overview

While supporting the goals and objectives of the hospital and upholding the ethics of the profession of pharmacy, theClinical Pharmacist is responsible for the delivery of pharmaceutical care and pharmacy services to patients, physicians,nurses and ancillary departments. The Clinical Pharmacist provides comprehensive pharmacy care throughprotocol/guideline management as well as individualizes drug therapies to promote safe, efficacious and cost-effectivepatient care. The Clinical Pharmacist serves as a role model for Staff Pharmacists, Pharmacy Residents, and pharmacystudents. The Clinical Pharmacist must contribute to the ongoing education and development of pharmacy personneland the education of other health care practitioners.

Responsibilities

ESSENTIAL FUNCTIONS:1. Collaborates with health care providers to individualize medication therapies to promote safe, efficacious, and cost-effective patient care following best practices and evidence-based guidelines.2. Contributes to formulary management and works with health care providers to ensure formulary compliance.3. Educates, mentors, and evaluates pharmacists, pharmacy residents, and/or pharmacy students to develop andimprove skills necessary for providing pharmaceutical care.4. Develops and provides educational programs, learning modules, and knowledge assessment tools.5. Educates other health care professionals regarding medications to promote optimal patient care via in-services,educational programs, face-to-face interactions, etc.6. Reviews, interprets, and evaluates all medication orders for patients in all age groups.7. Maintains patient medication profiles in an automated pharmacy information system.8. Consults with and advises medical, nursing, and other healthcare providers regarding the prescribing, dispensing, oradministration of medications.9. Routinely monitors the patient’s total drug therapy for effectiveness, side effects, adverse reactions, toxicities, druginteractions, and incompatibilities.10. Participates actively in cardiopulmonary emergencies.11. Conducts assigned target drug monitoring programs and concurrent drug use evaluation activities as required.12. Performs and documents Pharmacist Intervention activities such as recommendations on drug product selection,dosage adjustments, and appropriate drug therapy.13. Obtains medication histories on selected patients and communicates pertinent information to the appropriatehealthcare provider.14. Communicates with physicians, nurses, and other healthcare providers as necessary.15. Prepares and/or verifies the appropriate preparation of medications to be dispensed (including sterile preparations)using the correct technique, as required.16. Provides drug information and discharge counseling to patients, as required.17. Provides staffing coverage 24 hours per day, 7 days per week, as required.

Qualifications

MINIMUM QUALIFICATIONS:Education:Graduate of an accredited college of pharmacy with a Doctor of Pharmacy (PharmD) Degree.Experience: Completion an ASHP-accredited PGY-1 residencyORHave three or more years of pharmacy practice experience. Role model for pharmacy practice, as evidenced by demonstration of ongoing professionalism and contributionto the profession.Licensure or Certification: Licensure by the State of Arkansas Board of Pharmacy Specialties Certification Highly Preferred Completion of 30 hours of continuing education every two years.Skills: The Clinical Pharmacist must have training in on-line drug information resources, pharmacy automation, IVpreparation, and computer applications including Microsoft Word, Excel, and PowerPoint. Interpersonal and Communications Skills are required to maintain smooth cooperative working relationshipswith others. The Clinical Pharmacist must respond quickly to inquiries and exercise good listening skills. The Clinical Pharmacist must promote customer satisfaction as evidenced by positive interaction withcustomers, prompt resolution of problems, and recommendations for quality improvement.Physical Requirements: At various times, the following physical activities will be required: standing, carrying, walking, pushing, pulling,lifting, finger dexterity, talking, and hearing, sometimes exerting in excess of lifting 50 pounds and pushing 100-150 lbs. The sensory and communicative activities required for the position include feeling, hearing, seeing, smelling andspeaking. The Clinical Pharmacist is required to have the ability to see clearly at 20 or less inches, to see clearly at 20 feetor more, the ability to identify and distinguish colors, and to have the visual acuity to perform daily activities. The Clinical Pharmacist is subject to inside environmental conditions, hazards (including electrical currents andexposure to chemicals), and infectious diseases.
