• Position: In-Home RN/LPN
• Job Type: Part-Time only
• Pay: $43/hr - $45/hr
• Hours: 14 hours a week
• Schedule: 7 days a week, 2 hour shifts
• Location: Saginaw, MI

Responsibilities:

• Follow a Peristeen Bowel Program that is managed and monitored by the VA. 
• Conducting assessments and providing opportunities to residents to improve their well-being. 
• Works as a management team member representing Kennedy Care with facility manager and other housing management staff in serving residents/clients. 
• Provides limited case management (i.e., evaluation of social, psychological, and physical needs and the development of a service plan) for a resident.
• Monitors the delivery of services to residents to ensure they are appropriate, timely, and satisfactory by Kennedy Care Life Care Professionals. 
• Completes reports, scheduling of team members, and additional management functions as needed. 
• Complete assigned training and education as necessary
• Ability to use electronic software for timekeeping 
• Provide clear and efficient documentation

Requirements

• RN or LPN with a Michigan licensure – experience in acute hospital or skilled nursing preferred
• Valid Michigan Driver’s License
