Job Description:
The Social Service Assistant at Immaculate Mary Center plays a crucial role in supporting the social services department and contributing to the overall well-being of our residents. This position involves working closely with residents, their families, and the interdisciplinary team to ensure a holistic approach to care.
Responsibilities:
1.Resident Support: Provide emotional and social support to residents, fostering a positive and inclusive community within the nursing home.
2.Assessment and Documentation: Assist in conducting social assessments of residents and maintain accurate and up-to-date documentation of social service interventions.
3.Family Engagement: Collaborate with residents' families to address concerns, provide information, and involve them in care planning to enhance the overall resident experience.
4.Care Planning: Participate in care planning meetings, working closely with the interdisciplinary team to develop and implement individualized care plans that address the social and emotional needs of residents.
5.Advocacy: Advocate for residents' rights and assist in resolving concerns or issues related to their well-being.
6.Crisis Intervention: Provide support during times of crisis, working with residents and families to address emotional and social challenges.
7.Community Resources: Connect residents with relevant community resources and services to enhance their overall quality of life.
Qualifications:
-Experience in Social Work or related field preferred.
-Previous experience in a healthcare or long-term care setting is advantageous.
-Strong interpersonal and communication skills.
-Empathy and compassion for the elderly population.
-Knowledge of relevant regulations and ethical standards in social work.
