Qualifications of Home Health Medical Social Worker(MSW):

• Have a Master's degree from a school accredited by the Council on Social Work Education
• Has a Bachelor's Degree in social work, psychology, sociology or other field related to social work
• One year of Home Health experience
• Ability to exercise initiative and independent judgment
• Must be a team player

Responsibilities:

• Understand and adhere to established agency policies and procedures
• Understands and promotes principles of continuous performance improvement
• Acts as an consultant for the agency
• Serves as a resource person to employees, patients, physicians and other allied healthcare providers
• Develops a cooperative relationship and communicates effectively and professionally with physicians
• Maintains current knowledge of Medicare Conditions of Participation for social work services.
• Reports any accident, incident, lost articles or unusual occurrence to the Director of Nursing/Assistant of Director of Nursing
• Plans and coordinates all social services within the Agency
• Work with family
