Position Details
Organizes and conducts medically prescribed Occupational Therapy program in the home that will restore function, prevent disability, and promote rehabilitation.
Provides appropriate evaluation measures to assess individual rehabilitative needs.
Provides therapeutic intervention indicated by assessment. Evaluates response and progress made during Occupational Therapy treatment.
Provide Occupational Therapy visits to patients within their home/community setting for purposes of rehabilitation treatment.
Consult with RN Case Manager, therapy clinicians or patient's physician/allowed practitioner as need warrants.
